What should I bring to my appointment?

Personal Information:
- Your Social Security number or ITIN.
- Your spouse's full name and social security number.
- Date of birth and Social Security Card for dependents.
Other People on your Return:
- Date of Birth, Birth Certificate and Social Security Card.
- Childcare records (including the providers ID number) if applicable
- Amount of any alimony/child support paid and ex-spouse's Social Security number.
Education Payments/Health Care Verification:
- Forms 1098-T and 1098-E.
- 1095 Form provided by employer or 1095-A if health care was with Covered CA.
Other Financial Forms:
- If an Employee please bring all W-2 received.
- Self employed bring Forms 1099-MISC, Schedule K-1, income records to verify amounts not reported on 1099.
- Records of all expenses-check register or credit card statements and receipts-Totals.
- Property tax, interest, and insurance forms.
